We're so glad you are considering Morningside Animal Hospital for your pet's care. We’re excited to welcome you and your pet! To help us make your first visit as smooth and personalized as possible, we ask all new clients to complete a few steps before scheduling their first appointment. This gives our team the information we need to review your pet's history and prepare for their care.

1. Complete your Welcome Form

Tell us a little about you and your pet, including contact information, basic history, and a few important permissions we need to have on file before your first appointment.

2. Send us your pet's records

If your pet has seen another veterinarian, please send us their previous medical records. Recently adopted? Adoption paperwork is helpful, too.

3. Take care of your first-exam deposit

Your deposit reserves your pet's first exam with Morningside and will be applied toward that visit.

Once we've received and reviewed your information, we'll be in touch within 2 business days to schedule your first visit.
